Free Athletic Physicals

The Family Care Center, on the third floor of the Cloud County Hospital at 1100 Highland Dr, is again offering free athletic physicals to all Cloud County students entering grades seven through twelve. The physicals will be conducted on Wednesday, August 6th. Junior high physicals will begin at 5:00, and high school physicals will begin at 6:00 p.m. and continue until complete.
Students must bring a physical form with the first page completed by a parent or guardian, and signatures.The form is available at the CHS office, 436 West 10th, or at this link: Physical Form
